Sliding Fee Discount Program
What is it? Our Sliding Fee Discount Program is designed to provide reduced costs for psychiatry services based on your household size and income.
Who is eligible? Eligibility is determined based on the Federal Poverty Guidelines. Patients with a household income at or below 200% of the federal poverty level may qualify for a discount.
How to apply:
- Complete a Sliding Fee Discount Application available at the front desk.
- Provide proof of income (e.g., recent pay stubs, tax returns, or a letter from an employer).
- Submit your application and documents to the front desk.
